Healthy Smiles: Oral Health of Young Children Toolkit
This Toolkit is intended for CCHCs to use in training child care providers and parents/guardians on the Healthy Smiles curriculum. The Trainer's Guide contains detailed notes for leading a one-hour training session, and the Slide Presentation includes PowerPoint slides that accompany the training.

Consultation evaluation form (PDF download, 9kb). This form is intended for you to use in gathering feedback about your oral health consultation skills. This form may be a useful resource if you want to assess your consultation skills. The form should be given to the child care provider to fill out and return to you. The form does not have to be submitted to Healthy Smiles.
